Please upload a copy of your completed course assessment form. A completed course assessment form must have the signature of your Undergraduate Program Director, Graduate Program Director, Program Chair or Program Coordinator approving the courses that you are taking abroad. This form MUST be completed and uploaded 2 weeks prior to departure. It is possible to change your course selection once you arrive at your host institution, however, you must get sign off for any new courses added. A signature is required for each course you plan to take abroad. We encourage you to list more classes than you will actually take in case some are not available.
