Travel to Canada Documentation Request Form

Please complete this form if you are under the age of 18, traveling to Canada for your studies at York, and are unvaccinated.

At this time fully-vaccinated students no longer require a Covid-19 entry support letter, and as such they will no longer be issued.

Current Covid-19 Travel and Entry Requirements
-Students 18 years of age and older must be fully-vaccinated.
-Students who are not fully vaccinated and 18 years of age or older will be unable to enter Canada.
-Fully vaccinated students do not need to quarantine upon arrival at this time and do not need a quarantine plan.
-Students must upload their information to ArriveCan 72 hours prior to arrival to Canada. Vaccine certificates must be either in English or French or a certified translation.
-Students aged 17 who are unvaccinated or partially vaccinated will need to quarantine for 14 days. If you are a new student to York U and are enrolled in courses, you are eligible for quarantine accommodation arranged by York University.

Vaccine and Quarantine Information

Vaccines approved by Health Canada:

WHO Vaccines approved by the Government of Canada for the purposes of travel:

Note: Fully vaccinated for the purposes of travel to Canada means having received two doses of the approved COVID-19 vaccine + have waited 15 calendar days since your last shot before traveling. Ex. If you received your last shot of Moderna on August 2nd, you will be fully vaccinated on August 17th)

NOTE: The vaccine requirement for entry into Canada is different than what is considered fully vaccinated by the province and under the University’s vaccine guideline for those staying in Canada to live, work or study.

If you have a non-Health Canada approved vaccine, such as:

· COVAXIN

you must obtain a mRNA dose after arrival or have received 3 doses of the WHO-Emergency Approved vaccine to be considered fully vaccinated under the University’s guideline. The University is following guidance from the Ontario Ministry of Health and the Public Health Agency of Canada.
Do you need quarantine accommodations provided by York University? *

Do you need quarantine accommodations provided by York University? Yes- I am under the age of 18 and am NOT fully vaccinated and require accommodations for all 14 days of the mandatory quarantine. No- I am under the age of 18 and am NOT fully vaccinated. I will arrange my own quarantine accommodation that meets the requirements set out by the Canadian government. I understand I will need to file my plan with York University.

International students 18 years of age and older must be fully-vaccinated to travel to Canada. They don't require a quarantine plan.

York has an agreement with a hotel for those required to quarantine. If you stay at another hotel, you will be responsible for paying the costs and will not be reimbursed by York.

Entry support letters are no longer provided to fully-vaccinated students as they are not required at this time.
COURSE ENROLMENT

York University is booking quarantine accommodations for students who are enrolled in courses for the semester they are entering.

We will not be able to confirm your accommodations unless you are enrolled in courses..

We advise that you arrange your travel to follow, at minimum, 2 weeks after your course enrolment date in order to allow sufficient time to process your application. (For example, if your course enrolment date is April 1, arrange your travel for no earlier than April 15)

The University will only cover the cost of quarantine once.

The University will cover quarantine costs for students who have not been in Canada since Fall 2020 or are coming to Canada to study at York for the first time.

The University will not cover quarantine for those already in Canada who are leaving at their discretion for travel deemed non-essential. This includes leaving Canada for Reading Week, Winter break or at other times.
