University Health Insurance Plan (UHIP) Refund Request Form

The University Health Insurance Plan (UHIP) is a mandatory basic medical insurance plan for international students (akin to OHIP for domestic students). International students can only submit this form to request a refund if they demonstrate that they have alternate basic medical insurance comparable to UHIP. Application deadlines apply for the 2025-2026 year, as follows:

- Absence from Canada: March 1, 2026
- Diplomats: March 1, 2026
- Fall-only enrollment: March 1, 2026
- Foreign government-sponsored scholars: October 15, 2025 (fall students) or February 13, 2026 (winter-only students)
- Canadian/provincial government-insured students (ex. IFHP, OHIP, RAMQ, etc.): March 1, 2026 (if first insured on March 1, 2026 or earlier) or within the same month of enrollment if first insured on March 2, 2026 or later (ex. eligible for RAMQ as of March 2, 2026--the UHIP refund deadline is March 31, 2026; eligible for OHIP starting March 31, 2026--the UHIP refund deadline is March 31, 2026). No UHIP refunds allowed in August 2026.

By completing this form, you are requesting to be refunded the cost of UHIP (appearing in your student financial account with a "UHIP" notation) for the period during which you will be insured by an eligible alternate provider during the current policy year starting on September 1st and ending August 31st. Refund applications for previous academic years (ex, 2024-2025) will not be accepted.

DISCLAIMER: Domestic and international students cannot submit this form to opt out of extended health and dental insurance plans managed by the York Federation of Students, the York University Graduate Students' Association or the Canadian Union of Public Employees, Local 3903. (These fees are either billed separately to its members or appear in your student financial account with a YFS or GSA notation.) To opt out of these plans due to alternate extended health coverage, students must contact their union directly.
