All University Health Insurance Plan (UHIP) applications

The University Health Insurance Plan (UHIP) is a mandatory basic medical insurance plan for international students, trainees and visitors (akin to OHIP for domestic students). Please submit this form as follows:

- international and domestic students: if the UHIP Office invites you to submit this application form
- International Visiting Research Trainees (IVRTs): once the UHIP or IVRT Office invites you to submit this form
- visiting scholars: after consultation with the UHIP Office
- foreign-born dependants (spouse and/or children of the above students and researchers): after their arrival in Canada

Please note that if a dependant arrived in Canada more than 30 days after this form was submitted, the UHIP insurer will charge a $500 to enroll them. If a dependant's insurance is being renewed in the academic year(s) following their arrival and enrollment occurs after September 30th, the insurer will charge a $500 late fee to renew their insurance.
