SIN Clinic Appointment Booking Form (Summer 2025)

We have now reached capacity for booking appointments. We will be accepting walk-ins for SIN Clinic between 10:00 AM- 3:00 PM at York Lanes, Room 242. Please make sure you're bring the following documents if you're walking in without an appointment:

What to Bring: To apply for your SIN or make amendments to your record, you must bring the following original documents ONLY (NO COPIES)

Primary Document:
-For study permit holders: Study permit indicating "may accept employment" or "may work" in Canada.
-For work permit holders: Valid work permit
-Passport of your current Citizenship or Photo ID issued by the Canadian government (e.g., Ontario driver's license)

Please note that electronic copies, including photos on your phone, will not be accepted.
